What is a Proxy?
A Proxy is an agent or guardian that is authorized to represent the data subject. A copy of the Power of Attorney is required.
Anyone completing a DSAR Request for someone other than themselves is considered a Proxy. In order for the DSAR request to be valid, the Proxy must identify themselves by selecting Yes on the DSAR Request Form. The Proxy must provide the information below in order to submit their DSAR Request.
