Why does my Custom Question entry cause layout issues on Add Custom Question form?

Bad Entry.

Certain entries will cause issues with the Add Custom Question form. These Bad Entries most often stem from one of the following:
  1. Entries with long strings of text without any spaces.
  2. Entry Titles which exceed the 60 character limit.
  3. Entry Questions which exceed the 300 character limit.
  4. Phone numbers which exceed the 15 character limit.
If you're experiencing some sort of layout error on the Add Custom Question form, please check to make sure your entry doesn't fall into the above categories.
